The short answer
- Face in a bowl of cold water: 15 to 30 seconds, breath held, water around 10 to 15°C — cold, but not ice-bath cold. Repeat up to three times.
- Cold pack or cold wet cloth on eyes and cheeks: 30 seconds to 2 minutes.
- Cold pack on the sides of the neck: short applications of 15 to 30 seconds, repeated a few times.
- Splashing cold water: works in a pinch, but produces a weaker, briefer effect than immersion.
The effect starts within seconds and fades within minutes. It is a reset button for an acute spike of stress, not a way to change your baseline on its own.
What is actually happening
Cold water on the face triggers the diving reflex, a response humans share with seals and other diving mammals. Sensory receptors of the trigeminal nerve around the eyes, nose and forehead detect the cold, and the brainstem responds by increasing vagal output to the heart. Heart rate drops, and blood is shunted toward the brain and core.
Two details matter for doing it properly:
- The face is the trigger, not the body. Receptors concentrated around the eyes and nose drive the reflex. Cold on your hands or forearms does very little by comparison.
- Holding your breath strengthens it. In adults, the full dive response comes from the combination of face immersion and breath-holding. Cold alone produces a partial version.

How cold
Colder water produces a stronger response, up to a point. In a study of 50 healthy men, 30-second facial immersion in 10°C water produced marked slowing of the heart, while 25°C water and 35°C water did not — the warmer temperatures actually raised heart rate slightly. An older study of 23 volunteers found that the slowing was strongest with cold water, and stronger still when people had been sitting in a warm room beforehand.
Practical guidance used in dialectical behaviour therapy, where this technique is taught as a crisis skill, is to keep the water above 10°C (50°F). That is a sensible floor. Water much colder than that adds discomfort and cold-shock without adding much benefit for a short face dip.
Without a thermometer: fill a bowl with cold tap water and add a handful of ice. When it feels sharply cold on your fingers but you could keep your hand in for half a minute without pain, it is in range. Take the ice out before you dip.
One more finding is useful. In the older study, the response was larger after people had spent time in warm air. The reflex tends to be stronger when you start out warm — in an overheated room, or just after a hot shower.
How long
The research uses short exposures, and there is no evidence that longer is better:
- 15–30 seconds. The DBT protocol uses 30 seconds of face immersion with breath held. Facial immersion studies commonly use up to 30 seconds. Most people cannot comfortably hold their breath much longer than this with a cold face anyway.
- 16-second neck applications. A randomised trial of healthy adults tested cold packs applied in 16-second intervals to the lateral neck, the cheek and the forearm. The neck produced the clearest increase in heart rate variability and drop in heart rate, the cheek less, the forearm little.
- 2 minutes of cold compress. In a small study, a two-minute cold face mask applied before stressful arithmetic tasks slowed heart rate by around 20 to 27 percent at its peak and blunted the cortisol response compared with a control group. The sample was 25 young adults, so it is suggestive rather than settled.
Staying submerged longer, or using icier water, is not better. Once the reflex has engaged, extra time mostly adds cold stress. Two or three short rounds with normal breathing between them work better than one long one.
Five ways to do it, compared
- Face immersion in a bowl or sink. Strongest and most reliable. Needs privacy and a towel.
- Cold pack or bag of cold water on eyes and cheeks. Almost as effective, can be done lying down, and works at a desk. Wrap ice packs in a thin cloth so they do not burn the skin.
- Cold pack on the sides of the neck. Supported by the trial above and easy to do discreetly. Hold it just below the angle of the jaw, never pressing hard.
- Splashing. Quick and available anywhere with a tap. Splash repeatedly for 20 to 30 seconds rather than once.
- Cold shower or plunge. A different experience. Whole-body cold produces an initial cold-shock response — gasping, a surge of stress hormones — and the calm tends to come afterward. Useful for some people, but not the same as the targeted face reflex.
A 60-second protocol
For a sharp spike of stress or panic
- Prepare the bowl. Cold water, roughly 10 to 15°C. Towel within reach.
- Breathe normally, then take an ordinary breath in. Not a huge gulp, and do not hyperventilate first.
- Lower your face in so the water covers your forehead, eyes and nose. Hold for 15 to 30 seconds, or until you want to come up.
- Come up and breathe slowly. Four seconds in, six to eight seconds out, for three or four breaths.
- Repeat once or twice if needed.
- Follow with a longer exhale practice for a couple of minutes to extend the effect. Humming on the out-breath is a good option.
Who should skip it
Very cold water slows the heart quickly, which is the point — and also the risk. Talk to a doctor before trying this if you:
- have a heart condition, a history of arrhythmia, or have fainted unexpectedly
- have a naturally low resting heart rate or take medication that lowers it, such as beta-blockers
- have Raynaud's, a cold allergy, or another cold-sensitive condition
- have an eating disorder, which DBT guidance lists as a reason for medical advice first
Never practise breath-holding in a bathtub, pool, lake or any water you could slip into. A bowl at a sink is the only setting for this.
If you feel faint, dizzy or unwell, stop, sit down, and breathe normally.
Common questions
How long should you put cold water on your face to stimulate the vagus nerve?
Fifteen to thirty seconds of face immersion with your breath held is enough to trigger the diving reflex. A cold pack on the eyes and cheeks can be held for thirty seconds to two minutes. Two or three short rounds work better than one long one.
How cold should the water be for the diving reflex?
Around 10 to 15°C works well. Studies show clear slowing of the heart at 10°C, but not at 25°C or warmer. DBT guidance advises keeping water above 10°C (50°F).
Do you have to hold your breath?

For most healthy people, yes. Check with a doctor first if you have a heart condition, a low resting heart rate, take beta-blockers, have a cold-sensitive condition or an eating disorder. Never practise breath-holding in a bath, pool or open water.
